​​Step 3. Complete the Vaccine Form. Once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. Upon Completion of this form please take a picture or print out the last page for verification at your appointment. Failure to fill out this form will result in delays in your appointment.

​​​​​COVID-19 Vaccine Criteria 

Under the strict guidelines of the New York State Health Department, The Moderna Covid-19 Vaccinations are now available at Preventive Medical Care and we are now starting to vaccinate all eligible New Yorkers for Covid-19. In order to be able to get the vaccine please contact us and follow the following steps below in order to get an Appointment. For our vaccine, a second-dose appointment will be scheduled for your at the time you receive your first vaccine dose.  Appointments are not transferable.

Under subject please state if your are our patient, LIAC,  or LIALC.

​​​​Step 4.​ Show up to your appointment Please arrive at the vaccination site(Our office) ten minutes before your appointment. Be sure to bring the following info:​

1. A Government form of identification( License, Passport, etc)

2. Appointment documentation

​3. Printed NY State COVID-19 Vaccine Form.

Stay in your car, and follow the instructions given to you. At the end of your appointment you will be given a COVID-19 Vaccination Record Card. Please hold on to this card and bring it to your 2nd vaccination appointment. 

Note: if you have come into contact with someone who has Covid-19 or currently has covid-19 do not get the vaccine. You cannot get the vaccine if you are sick or exposed to somebody with covid-19

You will be required to schedule two vaccine appointments four weeks apart.

Are you eligible ?    We are currently vaccinating all eligible patients who are  over 18  for the vaccine.( Please refer to the "List of Comorbidities and Underlying Conditions" under eligible vaccine groups)   Not-eligible patients will be waitlisted.​ ​The full list of all eligible patient populations under New York State can be found below. ​(Note: new patient are required to have a physical before eligiblity).
